Roles & Responsibilities
- Provide compassionate, resident-centered nursing care in an assisted living setting
- Administer medications and treatments accurately and safely
- Perform assessments and document resident condition and care in a timely manner
- Collaborate with nursing staff, interdisciplinary team members, and leadership to ensure continuity of care
- Support residents with activities of daily living while promoting independence and dignity
- Adhere to infection control practices and participate in required health screenings
- Complete Trinity Specialty Specific Competency Testing and maintain required certifications
Qualifications
- Active, unencumbered LPN license in the state of practice
- BLS (Basic Life Support) certification required
- Successful completion of License/Certificate Verification
- Willingness to complete Trinity Specialty Specific Competency Testing
- Up-to-date immunizations and health screenings as listed: Fit Test, 2-step PPD or IGRA, Trinity 10 panel drug screen, respirator evaluation, MMR, Hepatitis B, Varicella, Tdap, COVID-19 vaccine, and seasonal influenza vaccine
- Comfort working independently in a per diem/pool model and adapting to variable schedules
Education
- Graduate of an accredited Practical Nursing program, or equivalent
- Current state LPN licensure required
Knowledge & Skills
- Strong clinical assessment and medication administration skills
- Experience with assisted living populations and geriatric care preferred
- Clear, concise documentation and charting abilities
- Good communication skills with residents, families, and care teams
- Infection prevention and control practices
- Ability to manage multiple priorities and respond calmly in urgent situations
- Willingness to learn facility-specific systems and complete competency testing
