Roles & Responsibilities
- Provide compassionate, skilled nursing care to residents in a Skilled Nursing Facility
- Administer medications and treatments safely and accurately according to policy
- Monitor and document resident condition, reporting changes to the care team
- Assist residents with activities of daily living and promote independence
- Participate in care planning and collaborate with nurses, therapists, and families
- Maintain accurate and timely electronic and paper health records
- Follow infection prevention protocols, including respirator and fit testing requirements
- Support a positive, patient-centered environment and advocate for resident needs
Qualifications
- Active LPN license (must be valid in Michigan for Michigan assignments)
- BLS (Basic Life Support) certification
- Experience in Skilled Nursing Facility (SNF) nursing or long-term care preferred
- Ability to join the per diem pool and work variable shifts as needed
- Successful completion of required health screenings and immunizations (PPD/IGRA, MMR, Hep B, Varicella, Tdap, COVID-19, Influenza)
- Ability to pass Trinity 10 Panel Drug Screen, respirator evaluation, and specialty competency testing
Education
- Graduated from an accredited LPN/LVN program (practical nursing diploma or certificate)
Knowledge & Skills
- Strong clinical assessment skills and sound judgment in acute and chronic care
- Familiarity with medication administration, wound care basics, and ADL support
- Comfortable with electronic medical records and timely documentation
- Team-player attitude with excellent communication skills
- Knowledge of infection control practices and respirator use
- Ability to adapt quickly to different units and resident populations
