Roles & Responsibilities

• Provide comprehensive inpatient clinical pharmacy services including order entry and order verification
• Accurately check and dispense medications to ensure patient safety
• Counsel patients and support safe, effective medication use across populations from pediatrics to geriatrics
• Collaborate with physicians, nurses, pharmacy technicians, and other care team members to optimize therapy
• Rotate into ambulatory roles as assigned such as anticoagulation clinic services or oncology pharmacy
• Ensure compliance with state and federal pharmacy laws and institutional policies
• Support continuous quality improvement and medication safety initiatives within the pharmacy

Qualifications

• Current license issued by the Pennsylvania State Board of Pharmacy or eligible for reciprocity in Pennsylvania within 90 days of hire
• Bachelor of Science in Pharmacy required; Doctor of Pharmacy preferred
• Recent hospital pharmacy experience preferred
• Pharmacy residency preferred for ambulatory rotations; advanced practice certifications a plus
• BLS certification required for anticoagulation clinic roles and must be renewed every two years
• Strong interpersonal and interprofessional communication skills

Education

• Bachelor of Science in Pharmacy required
• Doctor of Pharmacy (PharmD) preferred

Knowledge & Skills

• Medication safety best practices and clinical pharmacy services
• Order entry, order verification, medication checking, and medication dispensing
• Inpatient pharmacy workflows and ambulatory clinic operations
• Knowledge of state and federal pharmacy laws and regulatory compliance
• Patient counseling techniques across lifespan populations including geriatrics and pediatrics
• Ability to work collaboratively with multidisciplinary teams
