Roles & Responsibilities
- Assess and evaluate patients to identify movement dysfunction and pain
- Design and implement individualized treatment plans focused on therapeutic exercise and gait training
- Provide hands-on treatment and mobility training to improve independence and function
- Use pain management strategies and fall risk assessment to reduce adverse events
- Coordinate discharge planning to support safe transitions and positive patient outcomes
- Document evaluations, progress notes, daily notes and discharges thoroughly and timely
- Communicate regularly with providers, insurance companies, patients and the rehab team to support care plans
- Assist with maintaining a clean, safe treatment environment including treatment room upkeep and laundry
- Perform other duties as requested to support patient care and clinic operations
Qualifications
- Licensed Physical Therapist in Wisconsin or eligible for licensure in WI
- Strong experience with patient assessment, therapeutic exercise and gait training
- Demonstrated skill in pain management, fall risk assessment and discharge planning
- Excellent clinical documentation and clear communication skills
- Ability to collaborate with interdisciplinary teams and interact with patients and families professionally
- Current CPR/BLS certification or willingness to obtain
Education
- Doctor of Physical Therapy (DPT) or equivalent degree in Physical Therapy preferred
- Bachelor or Master in Physical Therapy considered when combined with relevant licensure and experience
