Roles & Responsibilities
- Evaluate patients thoroughly and use strong clinical reasoning to create individualized treatment plans
- Provide high-quality physical therapy services across inpatient, swing bed, outpatient and skilled nursing settings
- Document patient encounters and progress clearly and promptly in the EMR in accordance with regulatory and facility standards
- Collaborate closely with physicians, nurses and the interdisciplinary team to coordinate care and optimize outcomes
- Educate patients and their families on treatment plans, home programs and safe mobility
- Adapt care plans to meet the needs of diverse patient populations and diagnoses
- Participate in quality improvement initiatives to enhance patient care and departmental processes
- Maintain professional certification and licensure while engaging in ongoing professional development
Qualifications
- Active California Physical Therapy License in good standing
- Successful completion of the National Physical Therapy Examination (NPTE)
- Current AHA BLS (Basic Life Support) certification
- 1+ year experience in acute care, swing bed/skilled nursing, or outpatient settings
- Proficiency with EMR systems and basic computer skills
- Strong clinical reasoning, communication, and documentation abilities
- Attention to detail and strong problem-solving skills
- Commitment to ongoing professional development and teamwork
Education
- Bachelor's, Master's, or Doctor of Physical Therapy (DPT) from an accredited program
Knowledge & Skills
- Patient evaluation and individualized treatment planning
- Clinical documentation best practices and EMR proficiency
- Clinical reasoning and safe patient handling techniques
- Experience in acute care, swing bed, skilled nursing and outpatient environments
- Basic Life Support (BLS) knowledge and emergency awareness
- Clear communication with patients, families and interdisciplinary teams
- Problem-solving and attention to detail
- Comfort with computers and electronic charting systems
