Roles & Responsibilities
- Evaluate and treat residents in skilled nursing and post-acute settings to restore function and independence
- Develop and implement individualized treatment plans focused on meaningful outcomes
- Collaborate with an interdisciplinary team to coordinate comprehensive, patient-centered care
- Document accurately and timely in accordance with facility and regulatory standards
- Participate in care conferences and contribute clinical insights to care planning
- Lead or contribute to quality improvement efforts that enhance rehab outcomes
- Educate residents and families to support long-term recovery and safety
Qualifications
- Active California Physical Therapist license (required)
- Strong clinical reasoning and patient evaluation skills
- Excellent communication and documentation skills
- Skilled nursing or post-acute care experience preferred
- Dependable, flexible, and team-oriented with a commitment to compassionate care
- Ability to participate in interdisciplinary care conferences and quality initiatives
Education
- Graduation from an accredited Physical Therapy program (DPT or equivalent) preferred

- Patient assessment and rehabilitation techniques
- Treatment plan development and implementation
- Clinical documentation and regulatory compliance
