Roles & Responsibilities

• Perform thorough evaluations to assess patients physical abilities and develop individualized treatment plans
• Provide evidence-based physical therapy interventions to improve strength, mobility and function
• Educate patients and families on therapeutic exercises, safe movement and home modifications
• Collaborate with occupational therapists, nurses and physicians to coordinate comprehensive care
• Monitor patient progress and adjust treatment plans to achieve optimal outcomes
• Maintain accurate, timely documentation of assessments, progress notes and care plans in line with facility policy
• Ensure treatment plans comply with regulatory requirements and best practices in physical therapy

Qualifications

• Bachelor s, Master s or Doctoral degree in Physical Therapy from an accredited program
• Valid Physical Therapy license in the State of California (or ability to obtain one)
• Experience working in a skilled nursing facility or similar healthcare setting preferred
• Strong communication, interpersonal and problem solving skills
• Ability to work independently while also collaborating effectively with an interdisciplinary team
• Compassionate, patient focused approach and commitment to resident dignity

Education

• Bachelor s, Master s, or Doctoral degree in Physical Therapy from an accredited program
