Roles & Responsibilities
- Perform thorough evaluations to assess patients physical abilities and develop individualized treatment plans
- Provide evidence-based physical therapy interventions to improve strength, mobility and function
- Educate patients and families on therapeutic exercises, safe movement and home modifications
- Collaborate with occupational therapists, nurses and physicians to coordinate comprehensive care
- Monitor patient progress and adjust treatment plans to achieve optimal outcomes
- Maintain accurate, timely documentation of assessments, progress notes and care plans in line with facility policy
- Ensure treatment plans comply with regulatory requirements and best practices in physical therapy
Qualifications
- Bachelor s, Master s or Doctoral degree in Physical Therapy from an accredited program
- Valid Physical Therapy license in the State of California (or ability to obtain one)
- Experience working in a skilled nursing facility or similar healthcare setting preferred
- Strong communication, interpersonal and problem solving skills
- Ability to work independently while also collaborating effectively with an interdisciplinary team
- Compassionate, patient focused approach and commitment to resident dignity
Education
- Bachelor s, Master s, or Doctoral degree in Physical Therapy from an accredited program

- Patient evaluation and assessment

- Therapeutic modalities and adaptive equipment knowledge
