Roles & Responsibilities
- Provide compassionate, hands-on home health nursing care within the LPN/LVN scope of practice
- Administer medications, treatments, and wound care safely and accurately
- Document patient assessments and interventions thoroughly using Cerner
- Collaborate with the interdisciplinary team to coordinate care and achieve patient-centered goals
- Educate patients and families about care plans, medications, and home safety
- Monitor and report clinical changes promptly to ensure timely interventions
- Maintain BLS certification and follow infection control and safety protocols
Qualifications
- Active LPN/LVN license in the state of California (required)
- Minimum 2 years of clinical nursing experience (home health or post-acute strongly preferred)
- Current BLS (Basic Life Support) certification (required)
- Comfortable with Cerner charting and electronic documentation
- Experience with home health or post-acute care environments
- Able to complete a 12-hour orientation and meet required credentialing to submit
Education
- LPN/LVN diploma or vocational nursing certificate (required)
- BSN not required
Knowledge & Skills
- Strong clinical assessment and basic nursing skills
- Home health care practices and post-acute care knowledge
- Proficiency with Cerner electronic health record system
- Excellent communication and patient/family education abilities
- Time management and ability to work independently in patients’ homes
- Attention to detail for accurate documentation and medication administration
