Roles & Responsibilities

• Provide compassionate bedside care to assigned patients, prioritizing safety and dignity
• Administer medications and treatments accurately and document all care in the chart
• Monitor and report changes in patient condition promptly to the care team
• Collaborate with nurses, therapists, and providers to create and follow individualized care plans
• Perform routine assessments, wound care, and basic procedures per facility policy
• Educate patients and families on care, medications, and discharge instructions in clear, respectful language
• Maintain meticulous documentation and ensure regulatory compliance
• Actively contribute to a positive unit culture by communicating professionally and supporting teammates

Qualifications

• Current, active LPN license (ability to practice in New Mexico preferred/required)
• Fingerprinting must be completed prior to working on the floor — please plan ahead
• Previous experience in long-term care, rehab, or acute settings is a plus
• Basic Life Support (BLS) certification preferred; facility-specific certifications welcomed
• Strong clinical judgment, reliable attendance, and excellent communication skills

Education

• Practical Nursing diploma or certificate
• Active LPN license

Knowledge & Skills

• Skilled in medication administration and basic clinical procedures
• Comfortable with patient assessments, vitals monitoring, and documentation
• Ability to use electronic medical record systems and follow charting best practices
• Knowledge of infection control, safety protocols, and facility policies
• Excellent patient- and family-centered communication and teamwork
• Time management and prioritization on busy night shifts
