Roles & Responsibilities
- Provide direct and indirect patient care under the direction of an RN or LPN.
- Assist residents with activities of daily living (ADLs) including personal care and comfort.
- Deliver compassionate resident support that promotes dignity and independence.
- Maintain a safe, clean environment to promote resident well-being.
- Collaborate with nursing staff and the care team to support positive resident outcomes.
- Represent the facility with professionalism and a commitment to excellent patient experience.
Qualifications
- Active Illinois CNA license (must be active)
- Current American Heart Association BLS certification
- Minimum 2+ years of Long Term Care CNA experience (required)
- Up-to-date skills checklist (within 1 year) pertinent to Long Term Care — uploadable document
- Work history and 2 completed supervisory reference forms with dates employed (required)
- Applicants within 50 miles of the facility are not eligible; previous direct employees within the last 2 years are not eligible (facility will review cases individually)

- Patient care and personal care skills
- Resident support with compassion and respect
- Maintaining a safe and clean environment
- Comfortable working under RN/LPN direction in a long-term care setting
