Roles & Responsibilities

• Conduct thorough diagnostic assessments to identify functional needs and goals for adult patients
• Develop and implement individualized treatment plans that promote rehabilitation and independence
• Deliver high-quality occupational therapy services across the full range of SNF rehab cases
• Document patient evaluations, progress notes, and treatment plans clearly and promptly in Meditech EMR
• Collaborate with nurses, PTs, SLPs, physicians, and support staff to coordinate patient-centered care
• Supervise and manage OT tasks for the unit as needed and support continuity of care
• Adapt care and scheduling to meet operational needs, including occasional weekends and holidays

Qualifications

• Active Hawaii Occupational Therapy license required by assignment start (travelers must apply upon offer if not currently licensed)
• BLS certification required
• Minimum 2 years of OT experience working with adults in at least one of the following: Acute, SNF, Rehabilitation Hospital, or Outpatient settings
• Primary experience in SNF rehab preferred
• Comfortable using Meditech EMR and documenting patient care in an electronic system
• Ability to work collaboratively, manage a caseload, and adapt to variable scheduling

Knowledge & Skills

• Skilled in diagnostic assessments and clinical reasoning for adult rehab patients
• Strong treatment planning and therapeutic intervention skills
• Experience with rehabilitation equipment and techniques used in SNF settings
• Proficient in Meditech EMR documentation and workflow
• Good communication, interdisciplinary collaboration, and time management skills
• Patient-focused mindset with the ability to motivate and engage adults in therapy
