Roles & Responsibilities
- Coordinate patient care across disciplines to ensure timely, appropriate services
- Serve as the liaison between patients, families, nurses, physicians and the care team
- Lead interdisciplinary care planning and monitor care consistency with clinical needs
- Conduct utilization review and discuss resource needs and discharge plans with physicians
- Manage care transitions and ensure transfer of required patient information for continuity
- Collect and maintain regulatory and accreditation data to support compliance
- Analyze frequent emergency department utilizers and implement focused care strategies
- Consult with the healthcare team on complex patient issues to optimize outcomes
Qualifications
- Active Registered Nurse licensure in Hawaii (Hawaii license required)
- 2+ years of nursing experience (required)
- 6 months of Home Health experience (required)
- Travel/contract experience (required)
- Active and clear driver’s license and car insurance (required, documented)
- Homecare Home Base and OASIS experience (required)
- COVID-19 and influenza vaccination required (no declinations accepted)
- Bachelor’s degree preferred
- Prior Case Manager experience preferred
Education
- Registered Nurse (RN) licensure required
- Bachelor’s degree preferred (BSN preferred but not strictly required)
Knowledge & Skills
- Strong clinical assessment and care coordination skills
- Experience with discharge planning, utilization review and interdisciplinary collaboration
- Ability to collect, analyze and report data for regulatory and accreditation purposes
- Excellent communication and patient/family advocacy skills
- Proficiency documenting in electronic systems (Homecare Home Base / OASIS)
- Comfort managing complex patients, frequent ED utilizers, and diverse care needs
