Roles & Responsibilities
- Assess speech, language, cognitive-communication, and swallowing function for geriatric patients
- Develop individualized treatment plans that improve communication and quality of life
- Implement evidence-based therapy and adjust plans based on patient progress
- Collaborate closely with interdisciplinary teams including nursing, rehab, and families
- Document timely, clear, and thorough clinical notes in the facility EMR
- Provide education and training to caregivers and staff to support continuity of care
- Participate in care planning and discharge preparation to ensure smooth transitions
Qualifications
- State licensure in Speech Language Pathology or eligibility to obtain Georgia licensure
- ASHA CCC-SLP preferred or in progress
- Experience working with geriatric populations, dysphagia, and cognitive-communication disorders a strong plus
- Current CPR/BLS certification as required by facility
- Clean background check and ability to meet facility onboarding requirements
- Strong clinical reasoning, communication, and teamwork skills
Education
- Master's degree in Speech-Language Pathology (required)
Knowledge & Skills
- Skilled in dysphagia assessment and intervention
- Knowledgeable about geriatric communication and cognitive-communication disorders
- Proficient documentation and outcome measurement practices
- Comfortable working within interdisciplinary rehabilitation teams
- Ability to educate caregivers and support evidence-based practice
