Roles & Responsibilities
- Provide high-quality, compassionate nursing care to residents in the Hopkins Center
- Administer medications and treatments accurately and on time following facility protocols
- Assess, document and communicate changes in patient condition to the care team
- Collaborate with RNs, physicians and allied staff to develop and follow individualized care plans
- Support patients’ comfort and dignity while promoting safety and independence
- Maintain accurate clinical records and contribute to interdisciplinary care conferences
Qualifications
- Current Licensed Practical Nurse (LPN) credential required
- Active or eligible for Pennsylvania state licensure
- Current Basic Life Support (BLS/CPR) certification preferred or willing to obtain
- Strong clinical skills in medication administration, wound care and patient assessment
- Compassionate communicator who works well in a team and values patient-centered care
Education
- Vocational nursing diploma, certificate or equivalent LPN program completion
Knowledge & Skills
- Solid understanding of LPN scope of practice and nursing fundamentals
- Ability to accurately document in electronic and paper charting systems
- Good clinical judgment and effective prioritization during busy shifts
- Strong interpersonal skills for family communication and teamwork
- Infection control practices and safe medication administration
