Roles & Responsibilities
- Provide compassionate, high-quality bedside care as a Licensed Practical Nurse on the assigned unit
- Administer medications and treatments safely and accurately according to orders and facility policies
- Perform timely assessments, monitor vital signs, and report changes in patient condition
- Document patient care and charting clearly and professionally in the electronic medical record
- Collaborate with RNs, providers, and interdisciplinary team members to support excellent patient outcomes
- Support admissions, discharges, and transfers smoothly to keep unit flow efficient
- Maintain infection control standards and ensure patient safety at all times
Qualifications
- Active LPN license in Vermont or ability to obtain state licensure prior to start
- Current CPR/BLS certification (or willingness to obtain prior to start)
- Recent experience in a long-term care or skilled nursing setting preferred
- Able to pass background check, drug screen, and any required health screenings
- Professional, reliable, and committed to weekend coverage as outlined in the contract
Education
- Diploma or certificate from an accredited Practical/Vocational Nursing program
Knowledge & Skills
- Strong patient assessment and basic clinical skills (vital signs, wound care, ADL support)
- Medication administration and safe handling practices
- Clear, compassionate communication with patients, families, and the care team
- Comfortable using electronic medical records and documenting care accurately
- Ability to prioritize tasks in a busy clinical environment and work well under pressure
- Team-player mindset with a focus on patient-centered care
