Roles & Responsibilities

• Provide compassionate, high-quality bedside care as a Licensed Practical Nurse on the assigned unit
• Administer medications and treatments safely and accurately according to orders and facility policies
• Perform timely assessments, monitor vital signs, and report changes in patient condition
• Document patient care and charting clearly and professionally in the electronic medical record
• Collaborate with RNs, providers, and interdisciplinary team members to support excellent patient outcomes
• Support admissions, discharges, and transfers smoothly to keep unit flow efficient
• Maintain infection control standards and ensure patient safety at all times

Qualifications

• Active LPN license in Vermont or ability to obtain state licensure prior to start
• Current CPR/BLS certification (or willingness to obtain prior to start)
• Recent experience in a long-term care or skilled nursing setting preferred
• Able to pass background check, drug screen, and any required health screenings
• Professional, reliable, and committed to weekend coverage as outlined in the contract

Education

• Diploma or certificate from an accredited Practical/Vocational Nursing program

Knowledge & Skills

• Strong patient assessment and basic clinical skills (vital signs, wound care, ADL support)
• Medication administration and safe handling practices
• Clear, compassionate communication with patients, families, and the care team
• Comfortable using electronic medical records and documenting care accurately
• Ability to prioritize tasks in a busy clinical environment and work well under pressure
• Team-player mindset with a focus on patient-centered care
