Shift Type: Primarily Monday-Friday, 8:00 AM - 4:00 PM (5 x 8-hour shifts); some evenings and weekend coverage required — rotating day/evening shifts
Hours per Week: 40
Candidate Pay Rate: $67 per hour

Roles & Responsibilities
- Conduct thorough psychosocial assessments to identify patient and family needs
- Provide crisis intervention and supportive counseling in acute care settings
- Coordinate discharge planning and community referrals for safe transitions home
- Collaborate closely with ED and OB teams to address social service needs
- Document interventions and care plans accurately in the medical record
- Advocate for patients and families, connecting them with appropriate resources
- Participate in multidisciplinary rounds and contribute to care coordination
Qualifications
- Licensed Social Worker (LMSW or LCSW) in good standing — Hawaii licensure preferred or ability to obtain
- Clinical experience in acute care settings; ED and OB experience a strong plus
- Comfort with crisis intervention, discharge planning, and community referrals
- Excellent communication skills and the ability to work with diverse populations
- Ability to work some evenings and weekends as part of a rotational schedule
- Current professional liability coverage as required by employer/policy
Education
- BSW or MSW (Master of Social Work) — clinical degree strongly preferred
Knowledge & Skills
- Skilled in psychosocial assessment and evidence-based interventions
- Knowledge of acute care workflows and hospital systems
- Strong case management and care coordination abilities
- Proficient documentation skills and familiarity with electronic medical records
- Cultural competency and sensitivity to patient needs in a diverse community
- Team-focused mindset with the ability to collaborate across disciplines
