Roles & Responsibilities

• Conduct thorough psychosocial assessments to identify patient and family needs
• Provide crisis intervention and supportive counseling in acute care settings
• Coordinate discharge planning and community referrals for safe transitions home
• Collaborate closely with ED and OB teams to address social service needs
• Document interventions and care plans accurately in the medical record
• Advocate for patients and families, connecting them with appropriate resources
• Participate in multidisciplinary rounds and contribute to care coordination

Qualifications

• Licensed Social Worker (LMSW or LCSW) in good standing — Hawaii licensure preferred or ability to obtain
• Clinical experience in acute care settings; ED and OB experience a strong plus
• Comfort with crisis intervention, discharge planning, and community referrals
• Excellent communication skills and the ability to work with diverse populations
• Ability to work some evenings and weekends as part of a rotational schedule
• Current professional liability coverage as required by employer/policy

Education

• BSW or MSW (Master of Social Work) — clinical degree strongly preferred

Knowledge & Skills

• Skilled in psychosocial assessment and evidence-based interventions
• Knowledge of acute care workflows and hospital systems
• Strong case management and care coordination abilities
• Proficient documentation skills and familiarity with electronic medical records
• Cultural competency and sensitivity to patient needs in a diverse community
• Team-focused mindset with the ability to collaborate across disciplines
