Shift Type: Monday-Friday, 8:30 AM - 5:00 PM (Day shift) — Float/PRN visits as assigned; guaranteed 40 hours/week

Roles & Responsibilities
- Provide total direct nursing care for hospice patients within established standards of care, ensuring comfort and dignity
- Perform admissions and comprehensive assessments to create patient-centered care plans
- Manage cases and coordinate care across interdisciplinary teams and community partners
- Serve as a compassionate liaison between patients, families, referral sources, and the hospice team
- Conduct office-based clinical triage support and document visits accurately in the EMR
- Float to other sites as needed, delivering consistent, high-quality care across locations
- Maintain clear, timely documentation and follow agency timekeeping procedures
Qualifications
- 1+ years current Hospice LPN or Clinical Triage experience (required)
- Active Washington or Compact LPN license (required)
- BLS (AHA) certification (required)
- Valid driver’s license and current auto insurance (required)
- Ability and willingness to float to Oregon sites; Oregon RN license encouraged and application for OR licensure is required upon contract acceptance
- Acute/Med-Surg experience highly preferred
- Comfortable using WellSky Consolo EMR (preferred)
Education
- Licensed Practical Nurse (LPN) — active license required
- Graduate of an accredited practical/vocational nursing program or equivalent preferred
Knowledge & Skills
- Strong knowledge of hospice care principles and end-of-life symptom management
- Skilled in admission, assessment, and case management workflows
- Clinical triage and telephone assessment skills for office-based support
- Excellent patient and family communication and emotional support abilities
- Proficient documentation practices and familiarity with electronic medical records (WellSky Consolo preferred)
- Ability to prioritize, problem-solve, and adapt when floating between sites
