Roles & Responsibilities
- Provide high-quality, evidence-based physical therapy care for residents in a skilled nursing setting
- Assess patient function, establish measurable goals, and develop individualized treatment plans
- Deliver therapeutic interventions to improve mobility, strength, balance, and functional independence
- Collaborate with nursing, occupational therapy, speech therapy, and other care team members to coordinate patient-centered care
- Document patient progress accurately and timely in the medical record
- Educate patients, families, and staff on safe transfer techniques, fall prevention, and home exercise programs
- Participate in interdisciplinary team meetings to optimize outcomes and transitions of care
Qualifications
- Active Physical Therapist license in Texas (or compact privilege if applicable)
- Current BLS/CPR certification
- Experience in skilled nursing, long-term care, or geriatrics preferred
- Strong clinical assessment and intervention skills with an emphasis on functional outcomes
- Excellent communication and teamwork skills; able to collaborate across disciplines
- Ability to complete documentation and meet regulatory requirements in a timely manner
Education
- Doctor of Physical Therapy (DPT) or Master's in Physical Therapy (or equivalent professional degree)
Knowledge & Skills
- Skilled in therapeutic exercise, gait training, balance and fall prevention strategies
- Knowledge of rehab equipment and safe patient handling techniques
- Familiarity with long-term care regulations and documentation best practices
- Ability to create clear, outcome-focused treatment plans
- Strong problem-solving skills and clinical reasoning
- Comfortable working with an interdisciplinary team and mentoring support staff
