Roles & Responsibilities

• Provide high-quality, evidence-based physical therapy care for residents in a skilled nursing setting
• Assess patient function, establish measurable goals, and develop individualized treatment plans
• Deliver therapeutic interventions to improve mobility, strength, balance, and functional independence
• Collaborate with nursing, occupational therapy, speech therapy, and other care team members to coordinate patient-centered care
• Document patient progress accurately and timely in the medical record
• Educate patients, families, and staff on safe transfer techniques, fall prevention, and home exercise programs
• Participate in interdisciplinary team meetings to optimize outcomes and transitions of care

Qualifications

• Active Physical Therapist license in Texas (or compact privilege if applicable)
• Current BLS/CPR certification
• Experience in skilled nursing, long-term care, or geriatrics preferred
• Strong clinical assessment and intervention skills with an emphasis on functional outcomes
• Excellent communication and teamwork skills; able to collaborate across disciplines
• Ability to complete documentation and meet regulatory requirements in a timely manner

Education

• Doctor of Physical Therapy (DPT) or Master's in Physical Therapy (or equivalent professional degree)

Knowledge & Skills

• Skilled in therapeutic exercise, gait training, balance and fall prevention strategies
• Knowledge of rehab equipment and safe patient handling techniques
• Familiarity with long-term care regulations and documentation best practices
• Ability to create clear, outcome-focused treatment plans
• Strong problem-solving skills and clinical reasoning
• Comfortable working with an interdisciplinary team and mentoring support staff
