Roles & Responsibilities
- Conduct screening, clinical assessment, diagnosis, and treatment planning for clients from birth through elder
- Deliver brief therapy and set up continuing care to support lasting recovery and wellness
- Lead and co-facilitate patient care conferences and drive discharge planning for transitional placements
- Coordinate care with hospital teams and local community partners to secure the right services for each patient
- Provide client advocacy and assist with complex matters such as Power of Attorney when relevant
- Maintain accurate, timely documentation in electronic and paper records in line with state, federal and accreditation standards
- Support quality management initiatives, participate in trainings, and help integrate best practices into client care
- Assist with provider enrollment, billing duties assigned by lead clinical staff, and multidisciplinary collaboration
Qualifications
- Master's degree in Social Work, Psychology, Counseling or a related discipline from an accredited institution
- Licensed as, or actively pursuing licensure as, an LCSW, LPC, or LMFT
- Valid State of Alaska Driver's License or ability to obtain within six months of hire
- Must be 21 years of age
- Clinical competency required within 3 months of hire and every three years thereafter
- Demonstrated experience with discharge planning, care coordination, and case management (experience level discussed during interview)
Education
- Master's Degree in Social Work, Psychology, Counseling or related discipline
Knowledge & Skills
- Strong skills in discharge planning and care coordination across the continuum of care
- Proven ability to perform clinical assessments, screenings, diagnosis, and treatment planning
- Comfort with brief therapy techniques and appropriate referral practices
- Familiarity with documentation regulations, accreditation requirements, and hospital billing standards
- Ability to work effectively in multidisciplinary teams and collaborate with tribal health partners
- Cultural competency in Tribal health settings and sensitivity to community wellness approaches
- Experience with both electronic and paper medical records and basic provider enrollment/billing tasks
