Roles & Responsibilities

• Conduct comprehensive home health physical therapy assessments and create personalized treatment plans
• Deliver hands-on skilled therapy and progressive exercise programs to improve mobility and independence
• Educate patients and caregivers on safe transfers, fall prevention, and home exercise programs
• Document timely, accurate clinical notes and outcomes (Epic experience preferred) to support continuity of care
• Coordinate care with nurses, case managers, and other therapists to achieve patient-centered goals
• Perform equipment recommendations and home safety evaluations to optimize patient independence
• Participate in Trinity-specific competency testing and maintain skill checklists as required

Qualifications

• Active, verifiable Physical Therapy license/certification (license verification required)
• BLS (Basic Life Support) certification — required
• Home health or outpatient physical therapy experience preferred
• Epic experience preferred but not required
• Completion of Trinity Specialty Specific Competency Testing and Trinity Skills Checklist as part of onboarding
• Up-to-date immunizations and health screening: MMR, Hep B, Varicella, Tdap, COVID-19, Influenza, 2-step PPD or IGRA, and drug screening

Knowledge & Skills

• Solid knowledge of home health practice standards and Home Health Outcome measures
• Skilled in therapeutic exercise, manual therapy techniques, gait training, and balance training
• Strong clinical reasoning for discharge planning and functional goal setting
• Comfortable with home safety evaluations and adaptive equipment recommendations
• Clear, timely documentation and electronic health record proficiency (Epic preferred)
• Excellent communication and patient/caregiver education skills
