Roles & Responsibilities

• Conduct comprehensive psychosocial assessments that identify strengths, needs, and treatment priorities
• Develop individualized treatment plans grounded in evidence-based practice
• Deliver therapeutic interventions across the lifespan to address depression, anxiety, trauma, substance use, and adjustment disorders
• Collaborate closely with primary care providers, nurses, and behavioral health colleagues as part of an integrated care team
• Coordinate care and referrals to ensure patients receive holistic, whole-person services
• Maintain timely, clear, and compliant clinical documentation
• Provide culturally sensitive, trauma-informed care tailored to rural and underserved populations
• Participate in team meetings and care planning to improve patient outcomes and clinic workflows

Qualifications

• Active LISW license in Iowa (required)
• Master's degree in Social Work (MSW) from an accredited program (required)
• Minimum of 2 years of clinical experience preferred
• Experience working in rural or medically underserved settings is a plus
• Strong interpersonal and organizational skills with excellent clinical documentation
• Comfort collaborating in an integrated primary care/behavioral health team

Education

• Master's degree in Social Work (MSW) from an accredited program

Knowledge & Skills

• Expertise in psychosocial assessments and treatment planning
• Skilled in evidence-based therapeutic interventions (trauma-informed care, CBT, etc.)
• Experience treating depression, anxiety, substance use, and adjustment disorders
• Proficient clinical documentation and case management skills
• Care coordination and interdisciplinary collaboration
• Ability to engage patients across the lifespan and adapt interventions to rural populations
