Roles & Responsibilities
- Evaluate and treat patients with mobility and functional impairments to improve independence
- Develop and implement individualized treatment plans tailored to each patient
- Provide therapeutic exercises, gait training, and manual therapy to advance recovery
- Monitor patient progress and modify treatment plans as needed for best outcomes
- Maintain accurate and timely documentation to support continuity of care
- Collaborate with interdisciplinary healthcare teams to coordinate patient-centered care
Qualifications
- Active California PT license (required)
- Current BLS certification
- Previous SNF or rehab experience preferred
- Strong clinical judgment and excellent communication skills
- Available to start ASAP for this 13-week full-time assignment
