Candidate Pay Rate: $35 per hour

Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents of the long-term care facility
- Administer medications and treatments safely and accurately
- Monitor resident conditions and communicate changes to the care team
- Complete timely and thorough documentation in resident charts
- Assist with ADLs (activities of daily living) and support residents’ comfort and dignity
- Collaborate with nurses, therapists, and family members to create person-centered care plans
- Participate in shift handoffs to ensure continuity of care
Qualifications
- Valid Licensed Practical Nurse (LPN) license in the state of employment (or compact/licensure as applicable)
- Current BLS (Basic Life Support) certification preferred or required depending on facility policy
- Prior experience in long-term care, rehab, or skilled nursing preferred
- Strong clinical judgement, attention to detail, and excellent communication skills
- Ability to work independently on per diem shifts and collaborate as part of a multidisciplinary team
Education
- Graduate of an accredited practical nursing program (LPN diploma or certificate)
Knowledge & Skills
- Medication administration and documentation best practices
- Basic wound care and infection control procedures
- Familiarity with electronic medical records and charting
- Ability to assess changes in resident condition and escalate appropriately
- Compassionate resident-centered care and strong interpersonal skills
