Roles & Responsibilities
- Perform thorough evaluations to assess residents' physical abilities and needs
- Develop and implement individualized treatment plans that promote mobility and independence
- Deliver targeted physical therapy interventions and therapeutic exercises
- Educate residents and families on safe movement techniques and home modifications
- Collaborate with occupational therapists, nurses, and physicians to coordinate care
- Document assessments, progress notes, and treatment plans accurately and timely
- Monitor patient progress and adjust plans to optimize outcomes and safety
- Ensure care plans meet regulatory requirements and best practice standards
Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program
- Valid Physical Therapy license in California (or ability to obtain state licensure)
- Experience in a skilled nursing facility or similar healthcare setting preferred
- Strong communication, interpersonal, and problem-solving skills
- Ability to work independently and as part of an interdisciplinary team
- Compassionate, patient-centered approach to care
Education
- Bachelor's, Master's, or Doctoral degree in Physical Therapy (accredited program required)
Knowledge & Skills
- Skilled in patient evaluation, treatment planning, and documentation
- Proficient with physical therapy interventions and therapeutic exercises
- Familiarity with therapeutic modalities and adaptive equipment
- Experience coordinating care within a multidisciplinary team
- Strong clinical judgment, organization, and time-management skills
- Excellent communication and interpersonal abilities
