Roles & Responsibilities
- Evaluate patients with movement dysfunction or pain to determine functional limitations
- Develop and implement individualized treatment plans to improve mobility and independence
- Provide interventions aimed at pain management and fall prevention
- Document evaluations, progress notes, daily notes and discharges accurately and timely
- Coordinate with care team and assist with discharge planning to optimize patient outcomes
- Communicate with insurance companies, other providers and patients regarding care and approvals
- Maintain a clean, safe treatment environment — assist with treatment room cleaning and laundry
- Perform other duties as requested to support patient care and clinic operations
Qualifications
- Proven experience treating movement dysfunction, pain management and improving mobility
- Strong documentation skills including evaluations, progress notes and discharge summaries
- Comfortable communicating with providers, patients and insurance representatives
- Experience with fall prevention strategies and discharge planning
- Reliable, professional, and a team player who supports clinic operations
