Roles & Responsibilities
- Evaluate patients thoroughly and use clinical reasoning to determine functional limitations and goals
- Develop individualized treatment plans and update them based on patient progress
- Provide high-quality Physical Therapy services across inpatient, swing bed, outpatient, and skilled nursing settings
- Document assessments, plans, and progress accurately and timely in the EMR
- Collaborate with physicians, nurses, and the interdisciplinary team to coordinate patient care
- Educate patients and families on treatment plans, home programs, and safe discharge planning
- Ensure compliance with all California and federal PT regulatory requirements
- Engage in ongoing professional development and contribute to a culture of continuous improvement
Qualifications
- Active California Physical Therapy License in good standing
- Successful completion of the National Physical Therapy Examination (NPTE)
- Current AHA BLS (Basic Life Support) certification
- Bachelor's, Master's, or Doctor of Physical Therapy (DPT) from an accredited program
- 1+ year of experience in acute care, swing bed/skilled nursing, or outpatient settings
- Proficiency with computers and EMR systems
- Strong clinical reasoning, documentation, communication, and problem-solving skills
Education
- Bachelor's, Master's, or Doctor of Physical Therapy (DPT) from an accredited program
Knowledge & Skills
- Physical Therapy evaluation and treatment planning across multiple care settings
- Skilled inpatient, outpatient, swing bed, and skilled nursing care
- Accurate clinical documentation and EMR proficiency
- Patient management for diverse populations and diagnoses
- Strong communication, attention to detail, and interdisciplinary collaboration
