Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents consistent with plan of care
• Administer medications and treatments accurately and document thoroughly
• Monitor resident condition, recognize changes, and promptly communicate concerns to the care team
• Perform routine assessments, record vital signs, and maintain clear, timely records
• Support resident activities of daily living while promoting dignity and independence
• Collaborate with nursing staff, therapists and families to coordinate excellent care
• Help maintain a safe, clean and respectful environment for residents

Qualifications

• Must hold a current Licensed Practical Nurse (LPN) license — state licensure required
• Prior experience in a long-term care or skilled nursing setting is a plus
• Basic Life Support (BLS/CPR) certification preferred or willing to obtain
• Up-to-date immunizations and ability to pass background and health screenings
• Strong communication, teamwork and time-management skills

Education

• Graduate of an accredited practical nursing program; High school diploma or GED required

Knowledge & Skills

• Skilled in medication administration and basic wound care
• Comfortable with electronic and paper documentation systems
• Solid knowledge of infection control principles and resident safety practices
• Good clinical judgment, observation skills and ability to prioritize
• Personable bedside manner and ability to support families and residents with empathy
