Roles & Responsibilities
- Provide direct patient care with compassion and clinical accuracy
- Administer medications and treatments according to care plans and policy
- Monitor and document patient condition, reporting changes promptly
- Collaborate with nursing and rehab teams to support resident recovery
- Assist with activities of daily living while promoting dignity and independence
- Maintain accurate and timely clinical documentation in the medical record
- Support infection control and safety protocols to protect patients and staff
Qualifications
- Licensed Practical Nurse (LPN) credential required
- Current, unrestricted LPN license (state licensure required upon hire)
- Basic Life Support (BLS/CPR) certification required or obtained upon hire
- Prior experience in long-term care, rehab, or skilled nursing preferred
- Strong communication skills and ability to work well in interdisciplinary teams
- Comfortable with medication administration, wound care basics, and documentation
Education
- Vocational/Practical Nursing diploma or equivalent (LPN program)
Knowledge & Skills
- Knowledge of nursing principles and practical bedside care
- Ability to accurately document assessments and interventions
- Medication administration and basic pharmacology awareness
- Skilled in patient safety, infection control, and resident-centered care
- Good time management and prioritization in a busy clinical setting
- Compassionate communication with residents, families, and staff
