Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in the rehabilitation and long-term care setting
• Administer medications and treatments accurately and according to facility protocols
• Perform thorough assessments and document resident status, changes, and care delivered
• Collaborate with nurses, therapists, and interdisciplinary team members to support resident goals
• Support resident safety and dignity while fostering independence and quality of life
• Communicate clearly and empathetically with residents, families, and coworkers

Qualifications

• Active Licensed Practical Nurse (LPN) license — current and in good standing (required)
• Completion of an accredited LPN program or equivalent practical nursing education
• Current Basic Life Support (BLS/CPR) certification preferred or willingness to obtain
• Comfortable working in a long-term care/rehab environment and supporting geriatric populations
• Reliable, professional, and able to pass facility background and health screenings

Education

• Completed accredited LPN / Practical Nursing program (diploma or certificate)

Knowledge & Skills

• Strong clinical skills in medication administration and wound care
• Accurate documentation and charting abilities
• Good communication and interpersonal skills with residents and team members
• Ability to prioritize tasks and manage time during busy shifts
• Basic familiarity with electronic health records and point-of-care documentation
