Roles & Responsibilities

• Provide compassionate direct patient care to residents in a long-term care setting
• Administer medications and treatments accurately and document thoroughly
• Collaborate with nursing and interdisciplinary teams to implement care plans
• Monitor patient condition and communicate changes promptly to the care team
• Support residents with activities of daily living while promoting dignity and independence
• Participate in admission, discharge, and shift handoff processes to ensure continuity of care

Qualifications

• Current LPN license (must be valid in New Mexico)
• Basic Life Support (BLS/CPR) certification
• Previous experience in long-term care or rehabilitation preferred
• Strong communication and teamwork skills with a patient-centered approach
• Ability to pass required background and health screenings

Education

• Practical Nursing diploma or equivalent certification

• Medication administration and charting best practices
• Wound care basics and pressure injury prevention
• Vital signs assessment and recognizing clinical changes
• Electronic and paper documentation systems
• Time management and prioritization during busy shifts
• Empathy, de-escalation, and resident-focused communication
