Roles & Responsibilities

• Provide compassionate direct patient care and monitor vital signs
• Administer medications and treatments according to care plans
• Document patient status, interventions and responses accurately
• Collaborate with RNs, providers and interdisciplinary team to optimize care
• Educate patients and families on procedures, medications and discharge needs
• Assist with admissions, transfers and safe patient movement

Qualifications

• Current LPN license in New Mexico or eligible for state licensure
• Basic Life Support (BLS) certification or willingness to obtain quickly
• Clinical experience in long-term care or similar settings preferred
• Strong communication skills and ability to work well on a team
• Ability to cover scheduled days: Sunday, Friday and Saturday

Education

• LPN diploma or vocational nursing certificate

Knowledge & Skills

• Proficiency with medication administration and infection control practices
• Comfortable with charting and basic electronic health record tasks
• Solid assessment skills and ability to escalate changes in condition
• Time management for efficient care during extended day shifts
• Patient-centered approach and strong interpersonal skills
