Roles & Responsibilities
- Provide compassionate, high-quality bedside care to residents and patients
- Administer medications and treatments accurately and safely
- Perform thorough assessments and monitor vital signs, reporting changes promptly
- Document care and observations clearly in the patient record to support continuity
- Collaborate with interdisciplinary team members to create and follow personalized care plans
- Support resident dignity and comfort, building trusting relationships with patients and families
- Participate in shift handoffs and quality improvement efforts to enhance resident outcomes
Qualifications
- Active Licensed Practical Nurse (LPN) license in the state of practice (New Mexico) or ability to obtain quickly
- Current CPR/BLS certification
- Completion of an accredited practical nursing program
- Immunizations and health screenings per facility policy
- Comfortable working in long-term care or skilled nursing environments; strong clinical judgment and compassion
- Professional, reliable, and able to communicate effectively with patients, families, and the care team
Education
- Graduate of an accredited LPN/Vocational Nursing program
- High school diploma or equivalent
Knowledge & Skills
- Strong medication administration and documentation skills
- Skilled in basic wound care, vital sign monitoring, and ADL support
- Familiarity with electronic medical records and accurate charting
- Good assessment skills and the ability to escalate clinical concerns appropriately
- Team-focused attitude with excellent communication and time management
