Roles & Responsibilities
- Assess patients' mobility, strength, balance, and functional needs to build meaningful care plans
- Design and implement individualized therapeutic programs that help patients regain independence
- Monitor progress, adjust interventions, and document outcomes in the medical record clearly and promptly
- Collaborate with nurses, physicians, occupational therapists, and support staff to deliver coordinated care
- Educate patients and families on home exercise programs, safety strategies, and strategies for long-term recovery
- Participate in team huddles and contribute to quality improvement initiatives that enhance patient outcomes
Qualifications
- Licensed Physical Therapist in New Hampshire or eligible to obtain state licensure
- Doctor of Physical Therapy (DPT) preferred; Bachelor’s or Master’s in Physical Therapy considered
- Current BLS/CPR certification (or willingness to obtain prior to start)
- Experience in post-acute or rehabilitation settings preferred; strong clinical reasoning skills
- Excellent documentation skills and comfort using electronic medical records
- Strong communication and teamwork skills; patient-centered and compassionate approach
Education
- Doctor of Physical Therapy (DPT) preferred; or Bachelor’s/Master’s in Physical Therapy
Knowledge & Skills
- Skilled in patient assessment, therapeutic exercise, gait training, and functional mobility training
- Familiarity with common post-acute and rehab conditions including orthopedics and neurologic diagnoses
- Proficient in developing measurable, goal-oriented treatment plans and discharge planning
- Ability to educate patients and caregivers in plain language and motivate recovery
- Comfortable collaborating within an interdisciplinary care team and adapting to fast-paced settings
