Roles & Responsibilities

• Assess patients' mobility, strength, balance, and functional needs to build meaningful care plans
• Design and implement individualized therapeutic programs that help patients regain independence
• Monitor progress, adjust interventions, and document outcomes in the medical record clearly and promptly
• Collaborate with nurses, physicians, occupational therapists, and support staff to deliver coordinated care
• Educate patients and families on home exercise programs, safety strategies, and strategies for long-term recovery
• Participate in team huddles and contribute to quality improvement initiatives that enhance patient outcomes

Qualifications

• Licensed Physical Therapist in New Hampshire or eligible to obtain state licensure
• Doctor of Physical Therapy (DPT) preferred; Bachelor’s or Master’s in Physical Therapy considered
• Current BLS/CPR certification (or willingness to obtain prior to start)
• Experience in post-acute or rehabilitation settings preferred; strong clinical reasoning skills
• Excellent documentation skills and comfort using electronic medical records
• Strong communication and teamwork skills; patient-centered and compassionate approach

Education

• Doctor of Physical Therapy (DPT) preferred; or Bachelor’s/Master’s in Physical Therapy

Knowledge & Skills

• Skilled in patient assessment, therapeutic exercise, gait training, and functional mobility training
• Familiarity with common post-acute and rehab conditions including orthopedics and neurologic diagnoses
• Proficient in developing measurable, goal-oriented treatment plans and discharge planning
• Ability to educate patients and caregivers in plain language and motivate recovery
• Comfortable collaborating within an interdisciplinary care team and adapting to fast-paced settings
