Roles & Responsibilities

• Assess patients’ functional abilities and design individualized rehabilitation plans
• Deliver one-on-one therapeutic exercise, gait training, neuromuscular re-education, and manual techniques
• Use appropriate physical modalities to support patient recovery and comfort
• Provide compassionate education and motivation to help patients reach meaningful goals
• Document progress accurately and timely in the EMR (Cerner) and communicate effectively with the care team
• Adapt treatment plans for general orthopedics, vestibular/balance, and select neurological cases
• Collaborate with colleagues to ensure seamless, patient-centered outpatient care

Qualifications

• Active Illinois (IL) Physical Therapist License — must be active
• Current American Heart Association BLS (required)
• Minimum 1+ year of PT experience (required)
• Strong customer service focus with proven one-on-one outpatient care skills
• Experience with outpatient orthopedics, vestibular/balance, or neurological rehab preferred
• Ability to complete and provide a Skills Checklist (current to 1 year) and 2 supervisory references with dates employed (required)
• Applicants within 50 miles of the facility are not eligible — please confirm your permanent address as part of submission
• Candidates who were direct employees within the last 2 years are not eligible; facility will review on a case-by-case basis

Knowledge & Skills

• Therapeutic exercise prescription and progression
• Neuromuscular re-education techniques
• Physical modalities and clinical reasoning for modality use
• Gait training and balance/vestibular interventions
• Manual therapy techniques for outpatient populations
• Comfortable delivering one-on-one, patient-centered care
• Proficient documentation and EMR use (Cerner)
• Strong communication and interpersonal skills to motivate and educate patients
