Roles & Responsibilities

• Verify medication and IV orders to ensure appropriate drug selection and dosing
• Evaluate allergies, interactions, incompatibilities, and therapeutic drug levels
• Monitor and analyze therapeutic drug levels to optimize patient outcomes
• Oversee medication dispensing and supervise prescription filling for accuracy
• Provide patient counseling to ensure safe and effective medication use
• Perform and supervise computer data entry related to patient profiles
• Collect and analyze data for process improvement initiatives that improve safety and efficiency
• Deliver in-service education to community and support groups and participate in continuing education
• Serve as a professional role model and a resource for the health system team

Qualifications

• Graduate of an accredited Pharmacy Program
• Wisconsin Registered Pharmacist License required
• Experience with medication order review and IV medication orders
• Skilled in therapeutic drug monitoring and medication dispensing oversight
• Comfortable performing patient counseling and supervising pharmacy staff
• Proficient with computer data entry and patient profile systems
• Willingness to work night shifts and adapt to rotating schedules

Education

• Graduate of an accredited Pharmacy Program
