Roles & Responsibilities

• Develop and implement individualized rehabilitation plans to help patients regain function and independence
• Deliver therapeutic exercise, neuromuscular re-education, gait training, and manual techniques with skill and compassion
• Use clinical assessment to set measurable goals and track patient progress
• Collaborate with nurses, physicians, and interdisciplinary team members to optimize patient outcomes
• Educate patients and families on home programs, safety, and injury prevention
• Document timely, accurate patient records in Cerner and maintain required compliance documentation
• Coach and motivate patients to achieve meaningful improvements in mobility and quality of life

Qualifications

• Minimum 1+ year of Physical Therapy experience — required
• Active Illinois Physical Therapist (PT) license — must be current and active
• Current American Heart Association BLS (Basic Life Support) certification — required
• Completed work history and skills checklist (current to 1 year) pertinent to Physical Therapy
• Two supervisory reference forms completed with dates employed listed
• Applicants within 50 miles of the facility are not eligible; applicants who were direct employees within the last 2 years are not eligible (facility will run name clear case-by-case)

• Therapeutic exercise and rehabilitation planning

• Manual therapy and physical modalities
• Gait training and mobility assessment
• Strong interpersonal and effective communication skills
• Proficiency with Cerner electronic medical records
