Roles & Responsibilities
- Provide skilled physical therapy treatments under the direction of the supervising PT to improve patient mobility and function
- Implement individualized treatment plans and document progress clearly and promptly
- Assist with therapeutic exercises, gait training, transfers and use of assistive devices
- Collaborate with nurses, therapists and care staff to ensure coordinated, person-centered care
- Educate patients and families on home exercise programs and safe mobility techniques
- Monitor and report changes in patient condition to the supervising PT and clinical team
- Maintain accurate, timely documentation in the facility chart and therapy records
Qualifications
- Graduation from an accredited Physical Therapist Assistant program
- Current PTA license or certification in good standing (state licensure required or eligible to obtain)
- Basic Life Support (BLS) / CPR certification required or obtained upon hire
- Previous experience in a skilled nursing facility, long-term care, or outpatient setting is a plus
- Strong communication skills and a collaborative mindset
- Ability to lift and assist patients as required and follow safe patient handling procedures
Education
- Associate degree from an accredited Physical Therapist Assistant program
Knowledge & Skills
- Solid knowledge of therapeutic techniques, transfers and mobility training
- Familiarity with assistive devices and durable medical equipment
- Documentation and charting skills with attention to detail
- Patient education and motivational coaching skills
- Time management and the ability to prioritize caseloads effectively
- Comfort working as part of an interdisciplinary care team
