Roles & Responsibilities
- Provide compassionate direct care to residents, including assistance with activities of daily living
- Administer medications and treatments safely and accurately in accordance with orders
- Monitor and document vital signs, condition changes, and response to interventions
- Collaborate with interdisciplinary team members to support individualized care plans
- Complete Trinity specialty competency testing and skills checklists to maintain clinical readiness
- Perform routine assessments and report concerns to supervising RN or provider promptly
- Maintain accurate, timely documentation and ensure compliance with facility policies
- Support infection control practices and help maintain a safe, clean care environment
Qualifications
- Current Licensed Practical Nurse (LPN) license in Pennsylvania or compact/reciprocal eligibility
- BLS certification required
- ACLS certification required
- Successful license and certificate verification as part of onboarding
- Completion of Trinity Specialty Specific Competency Testing and Trinity Skills Checklist
- Up-to-date immunizations and health screenings per facility requirements (PPD/IGRA, MMR, Hep B, Varicella, Tdap, COVID-19, Influenza)
- Ability to pass Trinity 10-panel drug screen and required background checks
- Comfortable working in a per-diem pool with flexibility and professionalism
Education
- Graduate of an accredited Practical Nursing program or equivalent training
- Current LPN licensure required
Knowledge & Skills
- Knowledge of medication administration and safe nursing practices
- Strong clinical assessment and observation skills
- Familiarity with basic life support and advanced cardiac life support protocols
- Good communication and teamwork skills when interacting with residents, families, and staff
- Ability to follow established competency checklists and facility procedures
- Comfortable using electronic documentation and following standing orders
- Attention to detail for accurate charting and infection control adherence

This is a travel/contract role in Pennsylvania. You may also find charming small towns, farmers markets, and outdoor recreation in regions like the Poconos or Lancaster County for relaxed weekends off.
