Contract Type: Travel Contract
Shift Type: Per Diem (Flexible) — typically Monday-Saturday, 8:00 AM - 5:00 PM (day); may include occasional evenings and weekends as-needed

Roles & Responsibilities
- Provide skilled Licensed Practical Nurse care during home visits with compassion and professionalism
- Administer medications and treatments accurately and document care thoroughly
- Perform focused assessments and report changes in patient condition to the care team
- Deliver patient and family education to support safe, effective home care
- Maintain infection control standards and complete required health screenings
- Collaborate with interdisciplinary team members to coordinate personalized care plans
- Complete Trinity Specialty competency testing and required documentation in a timely manner
Qualifications
- Current LPN license (Licensed Practical Nurse) — Michigan licensure required or eligible
- BLS certification required
- Ability to pass License/Certificate Verification and Trinity Specialty Specific Competency Testing
- Meet health and screening requirements (Fit Test, 2-step PPD or IGRA, drug screen, MMR, Hep B, Varicella, Tdap, COVID-19, Influenza)
- Comfort working independently in patients' homes and managing documentation
- Home health experience preferred but new-to-home-health LPNs who meet requirements are encouraged to apply
Education
- LPN diploma, vocational certificate, or equivalent practical nursing education
Knowledge & Skills
- Strong clinical assessment and basic nursing skills (vitals, wound care, med administration)
- Clear, compassionate patient and family communication
- Accurate documentation and time management for home visit workflows
- Familiarity with infection control best practices and home safety assessments
- Ability to follow care plans and collaborate with a remote interdisciplinary team
- Comfort completing competency testing and adhering to agency protocols
