Contract Type: Travel Contract
Shift Type: Per diem pool; shifts vary and are scheduled as needed (days, evenings, weekends). Exact start/end times depend on assignment and will be provided at scheduling.

Roles & Responsibilities
- Provide compassionate, hands-on nursing care to program participants enrolled in PACE
- Administer medications and treatments safely and accurately according to policy
- Perform routine assessments and report changes in participant condition promptly
- Document care thoroughly in electronic and paper records to support continuity
- Collaborate with interdisciplinary team members to plan and coordinate participant care
- Support participant activities of daily living while promoting dignity and independence
- Complete required Trinity Skills Checklist and maintain compliance with facility processes
Qualifications
- Current, active LPN license (license verification required)
- BLS certification required
- Completion of Trinity Skills Checklist as part of onboarding
- Ability to pass required health screenings: fit test, 2-step PPD OR IGRA, MMR, Hepatitis B, Varicella, Tdap, COVID-19 vaccine, and annual influenza vaccine
- Successful completion of Trinity 10 Panel Drug Screen
- Strong communication skills and willingness to work collaboratively in a multidisciplinary team
- Previous experience in long-term care, geriatrics, home health, or PACE programs is a plus
Education
- Graduate of an accredited Practical Nursing program (LPN/LVN)
- High school diploma or equivalent
Knowledge & Skills
- Basic clinical assessment and vital signs monitoring
- Medication administration and documentation best practices
- Infection control and immunization protocols
- Patient-centered care planning and interdisciplinary teamwork
- Comfortable with electronic health record documentation
- Ability to prioritize tasks in a fast-paced, per diem environment
