Roles & Responsibilities
- Provide compassionate direct care to assisted living residents, focusing on dignity and comfort
- Administer medications and treatments accurately according to care plans
- Monitor resident status, document changes, and escalate concerns to RN leadership
- Support activities of daily living and promote resident independence
- Participate in competency testing and follow facility protocols for safety and infection control
- Collaborate with interdisciplinary team members to deliver coordinated care
- Complete required documentation and maintain license and certification compliance
Qualifications
- Licensed Practical Nurse (LPN) current and in good standing
- BLS certification required
- License/Certificate Verification will be completed prior to start
- Must meet facility health requirements: Fit Test, 2-step PPD or IGRA (Quantiferon/Tspot)
- Up-to-date immunizations as required: Influenza, MMR, Hepatitis B, Varicella, Tdap, COVID-19
- Trinity 10 Panel Drug Screen clearance and Trinity specialty competency testing
- Physical/Health Statement on file and any required respirator evaluations or color vision checks
Knowledge & Skills
- Strong clinical skills in medication administration and basic nursing assessments
- Knowledge of assisted living care needs and resident-centered approaches
- Infection prevention and control practices
- Ability to follow competency testing and facility-specific protocols
- Clear communication and teamwork skills with residents, families, and staff
- Reliable documentation and time management skills
