Roles & Responsibilities
- Provide compassionate direct patient care as an LPN in a long-term care setting
- Administer medications and treatments safely and accurately
- Perform timely assessments and document patient status in the medical record
- Collaborate with nurses, care teams, and leadership to support resident-centered care
- Follow infection control protocols and facility-specific competency requirements
- Complete required competency testing and maintain up-to-date certifications
- Communicate clearly with residents and families to promote understanding and comfort
- Support a positive, respectful culture that puts resident dignity first
Qualifications
- Current LPN license in Michigan (License/Certificate Verification required)
- BLS (Basic Life Support) certification required
- Completion of Trinity Specialty Specific Competency Testing as listed
- Ability to pass required health screenings and clearances (see checklist items)
- Current immunizations per facility policy: COVID-19, MMR, Hepatitis B, Influenza, Varicella, Tdap
- Successful completion of Fit Test, 2-step PPD or IGRA (Quantiferon/T-Spot), and a Trinity 10-panel drug screen
- Readiness to complete a physical/health statement prior to assignment
Education
- Graduate of an accredited LPN program
- Current LPN licensure in Michigan
Knowledge & Skills
- Strong clinical assessment and medication administration skills
- Comfortable with long-term care workflows and documentation
- Infection prevention and PPE best practices (including respirator evaluation/fit testing)
- Competency in specialty testing and facility-specific procedures
- Excellent communication and teamwork skills
- Ability to adapt quickly in a per diem environment and provide reliable coverage
