Roles & Responsibilities
- Assess patients in the home setting and evaluate clinical, functional, and emotional status
- Develop and periodically review individualized care plans with physicians, patients, and caregivers
- Coordinate care through clear interdisciplinary communication and timely case conferences
- Recertify patient care plans as appropriate and communicate changes to the care team
- Document evaluations, treatment goals, and progress thoroughly and professionally
- Teach patients and caregivers self-care techniques, medication management, and health counseling
- Communicate significant clinical changes to physicians and VNA colleagues to ensure safe transitions
- Advocate for patients and support discharge planning with other disciplines
Qualifications
- Current, unencumbered Registered Nurse license in Vermont and New Hampshire or a valid compact RN license
- Graduate of an accredited school of nursing
- Minimum 2 years of experience in Home Health
- Ability to work independently in the home setting while collaborating with an interdisciplinary team
Education
- Graduate of an accredited school of nursing (RN)

- Strong case management and home health nursing skills
- Care plan development and patient assessment expertise
- Excellent documentation and clinical charting abilities
- Patient and caregiver education skills with a teaching mindset
- Effective interdisciplinary communication and coordination
- Sound clinical judgment and time-management in community settings
