Roles & Responsibilities

• Utilize the nursing process to assess, plan, implement and evaluate individualized patient care
• Administer prescribed medications and treatments safely and in line with state laws and facility policy
• Document patient care and progress clearly and timely according to standards of care
• Develop, initiate and execute patient care plans in coordination with physicians and the care team
• Collaborate with interdisciplinary team members to support patients' longitudinal plan of care
• Participate in performance improvement and shared governance activities to advance unit quality
• Provide clinical guidance and support across units when floating is required due to census
• Apply critical thinking to promote patient safety and escalate concerns through the proper chain of command

Qualifications

• Active Registered Nurse (RN) license in Wisconsin (WI Nursing License) required
• Current CPR certification required
• Experience in skilled nursing or sub-acute care preferred
• Comfortable floating to other inpatient units as needed
• Strong documentation skills and familiarity with care planning and the nursing process
• Ability to work a 0.9 FTE day schedule with rotating weekends
• Excellent critical thinking, communication and interdisciplinary collaboration skills

• Medication administration and safe dosing practices
• Patient assessment and development of individualized care plans
• Accurate and timely patient documentation
• Care planning and execution in a skilled nursing setting
• CPR and basic life support knowledge
• Clinical decision-making with a focus on patient safety and experience
• Shared governance and participation in performance improvement initiatives
• Ability to collaborate across disciplines and support unit workflow when floating
