Roles & Responsibilities

• Provide high-quality physical therapy evaluations and evidence-based treatments for older adults
• Develop and implement individualized therapy plans that improve mobility, safety, and quality of life
• Document therapy services accurately and timely using Net Health
• Collaborate closely with nursing, rehab team members, and other healthcare partners to coordinate care
• Support a positive, resident-centered culture and contribute to interdisciplinary care planning
• Communicate progress and goals clearly with residents, families, and the care team
• Participate in staff meetings and contribute ideas to improve clinical processes and resident outcomes

Qualifications

• Valid Physical Therapist license in good standing (able to practice in the assignment state)
• CPR certification through the American Heart Association or the Red Cross
• Two-step PPD/TB skin test within 30 days of start date
• Covid vaccination records or documented exemption
• Background check and fingerprinting clearance
• Current resume and a recent physical performed by a physician, PA or nurse practitioner
• Experience working in senior care or with older adult populations preferred
• Strong communication, documentation, and teamwork skills

Knowledge & Skills

• Skilled in physical therapy evaluations and individualized treatment planning
• Proficient with clinical documentation best practices (Net Health experience a plus)
• Excellent interdisciplinary collaboration and communication skills
• Resident-centered care approach with attention to safety and mobility
• Ability to work in a fast-paced senior living environment and adapt to varied care settings
