Shift Type: Monday-Friday, 8:00 AM - 4:00 PM (Day shift)
Hours per Week: 40
Candidate Pay Rate: $53 per hour

Roles & Responsibilities
- Provide high-quality physical therapy evaluations and evidence-based treatments for older adults
- Develop and implement individualized therapy plans that improve mobility, safety, and quality of life
- Document therapy services accurately and timely using Net Health
- Collaborate closely with nursing, rehab team members, and other healthcare partners to coordinate care
- Support a positive, resident-centered culture and contribute to interdisciplinary care planning
- Communicate progress and goals clearly with residents, families, and the care team
- Participate in staff meetings and contribute ideas to improve clinical processes and resident outcomes
Qualifications
- Valid Physical Therapist license in good standing (able to practice in the assignment state)
- CPR certification through the American Heart Association or the Red Cross
- Two-step PPD/TB skin test within 30 days of start date
- Covid vaccination records or documented exemption
- Background check and fingerprinting clearance
- Current resume and a recent physical performed by a physician, PA or nurse practitioner
- Experience working in senior care or with older adult populations preferred
- Strong communication, documentation, and teamwork skills
Knowledge & Skills
- Skilled in physical therapy evaluations and individualized treatment planning
- Proficient with clinical documentation best practices (Net Health experience a plus)
- Excellent interdisciplinary collaboration and communication skills
- Resident-centered care approach with attention to safety and mobility
- Ability to work in a fast-paced senior living environment and adapt to varied care settings
