Candidate Pay Rate: $56 per hour

Roles & Responsibilities
- Evaluate patients in their home environment and create individualized plans of care
- Provide skilled occupational therapy treatment and adjust interventions as needed
- Perform and master CMS-required OASIS scoring and collaborate with Quality team on adjustments
- Coordinate care with physicians and the multidisciplinary team to ensure holistic care
- Document all skilled services, communications, and adverse events in the medical record
- Supervise Occupational Therapy Assistant services and complete required supervisory visits
- Manage productivity standards while maximizing continuity of care and efficient travel routes
- Maintain accurate time and mileage logs and follow agency commuting policies
- Participate in quality assurance and performance improvement initiatives
Qualifications
- Current Montana Occupational Therapy license (required)
- BS degree or higher from an accredited Occupational Therapy program
- Valid driver's license and appropriate auto insurance for home visits
- Two years of OT experience preferred
- Competence with CMS regulations and OASIS scoring
- Strong documentation skills and comfort with point-of-care charting
- Ability to collaborate effectively in multidisciplinary care conferences
- Willingness to participate in agency education and maintain CE requirements
Education
- BS degree or higher from an accredited Occupational Therapy program
Knowledge & Skills
- Occupational therapy evaluation and treatment planning in home health settings
- OASIS scoring and CMS/Medicare home health regulations
- Patient-centered care and family education techniques
- Documentation best practices and electronic medical records
- Supervision of OTAs and interdisciplinary communication
- Time and mileage management for home health caseloads
- Quality assurance processes and productivity standards
