Roles & Responsibilities

• Provide direct patient care including medication administration, assessments, and treatments with compassion and clinical precision
• Collaborate with interdisciplinary teams to create and update individualized care plans
• Monitor patient status and promptly report changes to the care team to ensure safety
• Document accurately and thoroughly in the facility EMR to maintain continuity of care
• Support patient and family education, helping them understand care plans and discharge instructions
• Participate in quality improvement initiatives to enhance patient outcomes and facility standards

Qualifications

• Active Licensed Practical Nurse (LPN) license in New Mexico (or ability to obtain state licensure prior to start)
• Basic Life Support (BLS) / CPR certification required
• Clinical experience in long-term care, rehabilitation, or similar settings is preferred
• Ability to pass facility onboarding, background check, and health screenings
• Strong communication skills and a collaborative approach to patient-centered care

Education

• Completion of an accredited Practical Nursing program and LPN licensure

Knowledge & Skills

• Proficient in medication administration and basic nursing assessments
• Familiarity with wound care, IV site monitoring, and common rehab nursing procedures
• Comfortable with electronic medical records and clear clinical documentation
• Good prioritization, time management, and teamwork skills
• Patient education and family communication to support positive outcomes
