Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents and patients during the PM shift
• Administer medications and treatments accurately and safely
• Assess and monitor patient condition; report changes promptly to the care team
• Document assessments, interventions and patient responses clearly in the medical record
• Collaborate with RNs, CNAs and interdisciplinary staff to deliver coordinated care
• Support patient and family education, promoting comfort, dignity and independence
• Maintain a safe, clean environment and follow infection control best practices

Qualifications

• Licensed Practical Nurse (LPN) — current, active license (Maryland license strongly preferred for this assignment)
• Graduate of an accredited LPN/Vocational Nursing program
• Basic Life Support (BLS) certification preferred or obtained upon hire
• Comfortable working the 3:00 PM - 11:30 PM shift and adaptable to facility scheduling
• Strong communication skills, attention to detail and a patient-first attitude

Education

• Graduate of an accredited LPN/Vocational Nursing program; High school diploma or equivalent

Knowledge & Skills

• Solid knowledge of medication administration and documentation practices
• Fundamentals of patient assessment, vital sign monitoring and wound care
• Familiarity with infection control, safety protocols and resident/patient rights
• Ability to use electronic health records and complete accurate charting
• Team collaboration, time management and the ability to prioritize in a busy setting
