Roles & Responsibilities

• Assess patient and caregiver needs using the nursing process to deliver high-quality, evidence-based home care
• Develop and communicate individualized plans of care that include patients and caregivers in decision making
• Serve as case manager: manage caseloads, coordinate visits, and meet agency productivity standards
• Complete OASIS documentation accurately and on time, staying current with requirement changes
• Implement medical plans and nursing interventions, ensuring safe, effective care and infection control
• Perform and maintain high-tech skills such as IV therapy, wound VAC management, and complex wound care
• Coordinate care with interdisciplinary team members, physicians, vendors, and community resources
• Educate patients and caregivers about medications, treatments, discharge instructions, and reportable conditions
• Delegate appropriately to LPNs within scope of practice and review documentation for quality
• Participate in peer review audits, case conferences, and quality improvement activities to support excellent care

Qualifications

• Active Registered Nurse (RN) license in Vermont or ability to obtain Vermont licensure
• OASIS experience required and strong documentation skills
• Home health nursing experience, including case management responsibilities
• Comfortable with IV therapy, wound VACs, and complex wound care
• Familiarity with McKesson processes or willingness to learn system workflows
• Ability to work collaboratively with LPNs and interdisciplinary teams
• Strong communication skills for patient/caregiver education and care coordination

• OASIS and OASIS documentation expertise
• Home health nursing assessments and plan of care development
• Case management and care coordination across disciplines
• Infection control best practices for home settings
• High-tech clinical skills: IV maintenance, wound VAC, complex wound care
• Familiarity with HHABN/Medicare forms and timely regulatory documentation
