Roles & Responsibilities

• Assess patients' mobility, strength, and functional needs and document results clearly
• Develop individualized treatment plans focused on meaningful, measurable outcomes
• Deliver hands-on therapy and therapeutic exercises that help patients regain independence
• Collaborate with nurses, physicians, and the rehab team to coordinate care
• Track progress, update care plans, and communicate results to patients and families
• Educate patients and caregivers on injury prevention and home exercise programs
• Maintain accurate documentation in a timely manner and adhere to facility policies

Qualifications

• Current Physical Therapist license (required) — ability to hold or obtain New Hampshire licensure is expected
• Active Basic Life Support (BLS/CPR) certification (or willingness to obtain)
• Clinical experience in inpatient or outpatient rehabilitation preferred
• Strong communication skills and ability to work collaboratively with interdisciplinary teams
• Comfortable treating a diverse caseload across all ages and diagnoses

Education

• Doctor of Physical Therapy (DPT) preferred or equivalent degree in Physical Therapy
• Bachelor's or Master's in a related field may be considered depending on credentials

Knowledge & Skills

• Proven knowledge of therapeutic modalities, manual therapy, and therapeutic exercise
• Skilled in patient evaluation, goal setting, and outcome tracking
• Good clinical decision-making and time management
• Familiarity with common documentation systems and compliance standards
• Strong interpersonal skills to motivate patients and work with families
