Roles & Responsibilities
- Assess patients' mobility, strength, and functional needs and document results clearly
- Develop individualized treatment plans focused on meaningful, measurable outcomes
- Deliver hands-on therapy and therapeutic exercises that help patients regain independence
- Collaborate with nurses, physicians, and the rehab team to coordinate care
- Track progress, update care plans, and communicate results to patients and families
- Educate patients and caregivers on injury prevention and home exercise programs
- Maintain accurate documentation in a timely manner and adhere to facility policies
Qualifications
- Current Physical Therapist license (required) — ability to hold or obtain New Hampshire licensure is expected
- Active Basic Life Support (BLS/CPR) certification (or willingness to obtain)
- Clinical experience in inpatient or outpatient rehabilitation preferred
- Strong communication skills and ability to work collaboratively with interdisciplinary teams
- Comfortable treating a diverse caseload across all ages and diagnoses
Education
- Doctor of Physical Therapy (DPT) preferred or equivalent degree in Physical Therapy
- Bachelor's or Master's in a related field may be considered depending on credentials
Knowledge & Skills
- Proven knowledge of therapeutic modalities, manual therapy, and therapeutic exercise
- Skilled in patient evaluation, goal setting, and outcome tracking
- Good clinical decision-making and time management
- Familiarity with common documentation systems and compliance standards
- Strong interpersonal skills to motivate patients and work with families
